Esbriet is an anti fibrotic drug used to treat idiopathic pulmonary fibrosis (IPF) by inhibiting pro fibrotic factors, reducing oxidative stress, and regulating inflammatory pathways. Its usage requires staged titration of dosage, and attention should be paid to monitoring liver function, preventing photosensitive reactions, and adjusting medication for special populations.

1. Usage and dosage

(1) Standard Dose Protocol

Day 1-7: 267mg (1 pill) 3 times a day; Day 8-14: 534mg (2 capsules) 3 times a day; From the 15th day onwards, maintain 801mg (3 pills) three times a day, taken with meals to reduce gastrointestinal reactions.

(2) Handling of missed services

After remembering, take it immediately and do not double the dosage for compensation. If it is close to the next medication time, skip it and continue taking the medication according to the original plan the next day.

(3) Dose adjustment

Abnormal liver function (ALT/AST>3 times ULN) requires reduction or discontinuation of medication; When combined with potent CYP1A2 inhibitors, reduce to 267mg three times a day; Treatment interruption of ≥ 14 days requires re titration.

2 Clinical efficacy

Pirfenidone can significantly slow down the annual decline rate of lung function (FVC reduction of about 50%) in IPF patients, reduce the risk of acute exacerbation, and improve quality of life. The therapeutic effect appears after 3-6 months of medication and requires long-term maintenance treatment.

3 Precautions

(1) Liver function monitoring

ALT/AST should be tested monthly before treatment and for the first 6 months. If symptoms such as jaundice and fatigue occur, immediate examination should be conducted. Severe liver damage (Child Pugh C grade) is contraindicated.

(2) Photosensitive protection

During treatment, it is necessary to use SPF50+sunscreen to avoid strong light exposure. If a severe photosensitive reaction occurs, the medication should be discontinued.

(3) Special populations

Pregnant women are prohibited (lack of safety data); Weighing the pros and cons of breastfeeding; Dialysis is not recommended for patients with end-stage renal disease; Smokers need to quit smoking to ensure effectiveness.

(4) Adverse reaction management

Common gastrointestinal reactions such as nausea and diarrhea often occur in the early stages of treatment. Severe skin reactions (such as SJS) require permanent discontinuation of medication.
