Methylsulfamethoxazole tablets are a sympathetic hyperfunction agent that increases blood pressure by inhibiting the reuptake and inactivation of norepinephrine by peripheral nerve endings, enhancing sympathetic nervous tension. Clinically, it is mainly used to treat intrinsic hypotension, orthostatic hypotension, and hypotension during dialysis.

Methyl sulfamethoxazole tablets, trade name: Risumic, active ingredient in English is Amezinium Metilsulfate.

2、 Indications

Used to improve hypotension in the following situations: intrinsic hypotension, standing hypotension, and blood pressure drop during dialysis implementation.

3、 Specifications

The specification is 10mg per tablet.

4、 Main components

Each tablet contains 10mg of active ingredient: Aminium Metilsulfate.

5、 Usage and dosage

According to indications:

1. Intrinsic hypotension and standing hypotension: The usual dose for adults is 20mg per day, divided into 2 oral doses (10mg each time). It can be adjusted appropriately according to age and symptoms.

2. Improvement of blood pressure drop during dialysis: The usual dose for adults is 10mg orally at the beginning of dialysis. It can be adjusted according to age and symptoms.

Doctors should flexibly adjust the dosage of medication based on age and symptoms.

7、 Medication precautions

1. Before and after meals: Regular oral administration.

2. Omission treatment: Medication should be taken according to the doctor's instructions, and should not be increased or decreased arbitrarily.

3. Vomiting treatment: If vomiting occurs, it is recommended to consult a physician or pharmacist.

4. Other important reminders: PTP packaged drugs should be taken after removing the PTP tablets to prevent serious complications such as esophageal perforation caused by swallowing hard acute angle tablets.

8、 Medication for special populations

1、Pregnant women: Use only when the benefits of treatment outweigh the risks.

2、 Breastfeeding women: Considering the benefits of treatment and breastfeeding, decide to continue or discontinue breastfeeding (animal experiments have shown that it can be transferred to breast milk).

3、 Children, etc.: No clinical trials have been conducted for infants and young children.

4、 Elderly people: Pay attention to starting with low doses, as physiological functions (kidneys, liver) are often low.

5、Attention for patients with comorbidities:

Severe heart disease patients: may worsen heart disease due to overactive sympathetic nervous system function.

Patients with hypertension, hyperthyroidism, pheochromocytoma/paraganglioma, angle closure glaucoma, and prostate enlargement with residual urine are contraindicated (see contraindications for details).

9、 Adverse reactions

The following adverse reactions may occur and need to be observed thoroughly. If there are abnormalities, they should be stopped and handled appropriately:

1. Less than 0.1% to 5%: rash, eczema, urticaria (allergy); Palpitations, tachycardia, changes in blood pressure, arrhythmia (premature contractions, atrial fibrillation, etc.), hot flashes, and a feeling of upwelling; Dizziness, blackening of the eyes, headache, heaviness of the head, and discomfort.

2. Less than 0.1%: chest discomfort, difficulty breathing, edema, chest tightness; Cold sensation in limbs; Stuck gait, fatigue, restlessness, emotional instability, insomnia, drowsiness, general numbness, tinnitus.

3. Unknown frequency: nausea, vomiting, loss of appetite, stomach discomfort, stomach pain, abdominal pain, diarrhea, constipation, dry mouth; Abnormal liver function and jaundice.

10、 Contraindications (prohibited for the following patients)

1. Hypertensive patients (can worsen hypertension).

2. Patients with hyperthyroidism (which can worsen hyperthyroidism).

3. Patients with pheochromocytoma or paraganglioma (which may cause sudden episodes of hypertension).

4. Patients with angle closure glaucoma (which may cause a sharp increase in intraocular pressure).

5. Patients with prostate enlargement accompanied by residual urine (which may cause urinary obstruction).

11、 Drug interactions

Attention should be paid to the combined use:

1. Drug name: Droxidopa: May cause abnormal elevation of blood pressure. Mechanism: This drug inhibits the reuptake and inactivation of norepinephrine converted from levodopa at the peripheral nerve endings.

2. Drug name: Noradrenaline: May cause abnormal elevation of blood pressure. Mechanism: This drug inhibits the reuptake and inactivation of norepinephrine at the peripheral nerve endings.
