Nausea, diarrhea, and cytopenias are common during azacitidine treatment. Scientific management can improve tolerability and treatment continuity.

I. Nausea Management

1. Prophylactic Antiemetics

Use antiemetics before dosing as directed by the physician, which can significantly reduce the occurrence of nausea; if the regimen includes oral tablets, take them with food to reduce stomach discomfort.

2. Dietary Adjustment

Eat small, frequent meals; choose light, easily digestible foods; avoid greasy, spicy, and strongly odored foods; cold foods have less odor and are easier to accept.

3. Timely Reporting

Persistent vomiting can cause dehydration or electrolyte imbalance. If nausea affects eating or vomiting occurs, inform the physician promptly to adjust the antiemetic regimen; do not stop medication on your own.

II. Diarrhea Management

1. Fluid Replacement to Prevent Dehydration

During diarrhea, pay attention to replenishing water and electrolytes; oral rehydration salts may be consumed; monitor urine output—reduced urine suggests dehydration and requires prompt medical attention.

2. Diet and Medication

Avoid raw and cold foods, high-fiber foods, and dairy products; use antidiarrheal agents as directed by the physician when necessary; record the frequency and character of bowel movements to facilitate the physician's assessment.

3. Watch for Infection and Bleeding

If accompanied by fever, bloody stool, or severe abdominal pain, seek medical attention immediately; do not self-administer antidiarrheal agents to mask symptoms.

III. Cytopenia Management

1. Regular Monitoring

Check blood counts regularly before and after treatment, focusing on changes in neutrophils, platelets, and hemoglobin; the physician will adjust the dose or postpone administration accordingly.

2. Infection Prevention

When neutrophils are reduced, pay attention to hand hygiene and avoid crowded places; if fever occurs (especially body temperature ≥38°C), seek medical attention immediately; do not self-administer antipyretics.

3. Bleeding Prevention

When platelets are reduced, avoid strenuous activities and collisions; use a soft-bristled toothbrush; promptly report gum bleeding, subcutaneous bruising, black stool, etc.

4. Managing Anemia

Decreased hemoglobin can cause fatigue and shortness of breath; ensure adequate rest and, if necessary, receive blood transfusion or hematopoietic agents as directed by the physician.
