Semaglutide tablets effectively control blood glucose levels by simulating the mechanism of human intestinal insulinotropic action, enhancing insulin secretion, inhibiting glucagon secretion, and delaying gastric emptying.

2、 Indications
1. As an auxiliary treatment of diet and exercise, it is used to improve the blood sugar control of adult type 2 diabetes patients.
2. Restricted use:
Not recommended as the preferred treatment for patients with poor dietary and exercise control.
Not studied in patients with a history of pancreatitis.
It is not suitable for the treatment of type 1 diabetes or diabetes ketoacidosis.

5、 Usage and dosage
1. Initial dose: 3mg once daily for 30 days.
2. Maintenance dose: Increase to 7mg once daily after 30 days.
3. Enhanced dosage: If additional blood glucose control is required, it can be increased to 14mg once daily after 30 days.
4. Key usage:
Take on an empty stomach in the morning, and fast for at least 30 minutes before and after taking the medication.
Take only ≤ 120mL of clean water and do not chew/break the entire tablet when swallowed.
6、 Dose adjustment
1. Omission treatment: skip the daily dose and take the medication according to the original plan the next day.
2. Conversion therapy:
Convert RYBELSUS14mg to OZEMPIC ® Injection: Start 0.5mg once a week the next day.
From OZEMPIC ® 0.5mg to RYBELSUS: 7mg or 14mg should be started within 7 days after the last injection.
7、 Medication precautions
1. Medication time: It must be taken on an empty stomach, and taking it with food/drink will reduce its effectiveness.
2. Gastrointestinal reactions: common nausea, vomiting, and diarrhea, often occurring during the dose escalation period.
3. Drug interactions:
When used in combination with insulin or sulfonylureas, the dosage needs to be adjusted to prevent hypoglycemia.
Delayed gastric emptying may affect the absorption of other oral medications (such as levothyroxine, which needs to be monitored).
8、 Medication for special populations
1. Pregnant women: Discontinue medication at least 2 months before pregnancy (with a long half-life).
2. Breastfeeding period: Prohibited (drugs may be secreted through breast milk).
3. Renal insufficiency: No need to adjust dosage.
4. Liver dysfunction: No need to adjust dosage.
5. Elderly: Age does not affect pharmacokinetics.
9、 Adverse reactions
1. Common (≥ 5%): Nausea (20%), abdominal pain (11%), diarrhea (10%), decreased appetite (9%), vomiting (8%), constipation (5%).
2. Serious adverse reactions:
Risk of thyroid C-cell tumors (animal experiments).
Pancreatitis (incidence rate 0.1%/year).

11、 Drug interactions
Caution should be exercised when using:
1. Insulin or sulfonylureas (increase the risk of hypoglycemia).
2. Oral medications with narrow treatment windows (such as warfarin and digoxin).
3. Levothyronine: Exposure increases by 33% when used in combination.
