Abemaciclib has a higher risk of use in specific populations and requires strict evaluation of its benefits and risks. The following three groups of people need to pay special attention to medication safety and follow corresponding management principles.

1. Patients with severe liver dysfunction

1.1 Prohibition principle

Patients with severe liver function impairment (Child Pugh C grade) should not use Abemaciclib, as impaired drug metabolism can significantly increase blood drug concentrations and increase the risk of toxicity.

1.2 Moderate Damage Adjustment

Patients with moderate liver function impairment (Child Pugh B grade) should reduce the dosage under the guidance of a doctor and closely monitor liver enzymes and bilirubin levels. Medication should be suspended when deterioration occurs.

2. Patients with severe renal insufficiency

2.1 End stage renal disease is contraindicated

Patients with end-stage renal disease requiring dialysis (eGFR<15mL/min/1.73m ²) are not recommended to use it due to lack of safety data and high risk of drug accumulation.

2.2 Use with caution for moderate to severe damage

Patients with moderate to severe renal insufficiency should increase the frequency of blood routine and liver function monitoring, and reduce or discontinue medication when serious adverse reactions occur.

3. Pregnant and lactating patients

3.1 Prohibited during pregnancy

Animal studies have shown that Abemaciclib has embryonic fetal toxicity, and its use during pregnancy may cause harm to the fetus. Women of childbearing age must take effective contraceptive measures during treatment and for at least 3 weeks after discontinuing medication.

3.2 Confirmation of pregnancy status before treatment

Before starting treatment, a pregnancy test must be conducted to confirm a negative result before initiating the course of treatment.

3.3 Breastfeeding is prohibited during lactation

Medications may be secreted into breast milk, posing potential risks to infants. Breastfeeding should be stopped during the treatment period and at least 3 weeks after discontinuation of medication.

4. Other alert groups

4.1 Individuals with a history of interstitial lung disease

Individuals with a history of interstitial lung disease or pulmonary fibrosis should undergo a thorough risk assessment before use.

4.2 Fertility Impact Notification

Men and women with fertility should be aware that this medication may affect fertility, and it is recommended to consult a reproductive specialist before treatment.
