During the treatment of Dasatinib, attention should be paid to four more serious adverse reactions: pleural effusion, QT interval prolongation, risk of bleeding, and pulmonary arterial hypertension. Early identification and standardized management are the core links to ensure medication safety.

1、 Monitoring and treatment of pleural effusion

Regular evaluation

Proactively report any difficulty breathing, dry cough, chest pain, or rapid weight gain during each follow-up visit.

imaging examination

The doctor will arrange a chest X-ray or ultrasound based on the symptoms to determine the degree of fluid accumulation.

hierarchical management

Mild fluid accumulation can continue to be treated with medication and strengthened monitoring; Moderate to severe fluid accumulation requires suspension of dasatinib, and if necessary, diuretics or thoracentesis drainage should be administered. After relief, the dosage should be reduced and recovery should be achieved.

2、 Prevention of QT interval prolongation

Baseline and Review

Regularly perform electrocardiograms before medication and during treatment, especially for those with concomitant bradycardia and electrolyte imbalances.

Be cautious of other QT inducing drugs

Avoid co administration with drugs known to prolong the QT interval, such as antiarrhythmic drugs and quinolones.

Timely correction of electrolytes

Maintain normal levels of blood potassium and magnesium to avoid increasing the risk of arrhythmia due to low potassium or low magnesium.

3、 Prevention of bleeding risk

Notify of coagulation history

Individuals with a tendency towards bleeding or a recent history of surgery should inform their doctor in advance.

Avoid using anticoagulant/antiplatelet drugs in combination

When not necessary, avoid simultaneous use with warfarin, aspirin, etc. If combined use is necessary, strengthen bleeding monitoring.

Observe bleeding symptoms

If skin bruising, gum bleeding, black stool or hematuria occur, seek medical attention immediately.

4、 Identification of pulmonary arterial hypertension

Be alert to related symptoms

If there is progressive shortness of breath, decreased activity tolerance, lower limb edema or fainting, it is necessary to inform the doctor in a timely manner.

Cardiac ultrasound screening

For symptomatic or high-risk patients, doctors will arrange echocardiography to evaluate pulmonary artery pressure.

Discontinuation of medication and referral

After being diagnosed with pulmonary arterial hypertension, dasatinib is usually discontinued and referred to a cardiac specialist for further treatment.
