Precautions for taking rivaroxaban: bleeding risk, liver and kidney function monitoring, and drug interactions

Rivaroxaban is a commonly used oral anticoagulant for the prevention and treatment of thrombotic diseases. During medication, special attention should be paid to the risk of bleeding, changes in liver and kidney function, and drug interactions to ensure efficacy and safety.

1、 Identification and management of bleeding risk

1. Bleeding is the main risk

Rivaroxaban exerts anticoagulant effects by inhibiting coagulation factors, and bleeding can occur in any part of the body, which can be life-threatening in severe cases.

2. Common bleeding manifestations

Mild bleeding includes skin bruising, gum bleeding, and nosebleeds. Severe bleeding includes black stool, vomiting blood, hematuria, hemoptysis, intracranial hemorrhage, etc.

3. High risk population

Elderly people, low weight individuals, those with renal insufficiency, and those with a history of bleeding or peptic ulcers have a higher risk of bleeding.

4. Daily prevention

5. Immediate medical attention

When vomiting blood, black stool, hematuria, severe headache, altered consciousness, or unstoppable bleeding occurs, medication should be stopped immediately and emergency medical treatment should be sought.

6. Do not discontinue medication on your own

Except for severe bleeding, rivaroxaban should not be discontinued on its own to avoid increasing the risk of thrombosis. Any adjustments require evaluation by a doctor.

2、 Liver and kidney function monitoring

1. Liver function monitoring

Rivaroxaban is partially metabolized by the liver. Severe liver dysfunction with coagulation disorders is contraindicated. Liver function should be tested before and during medication.

2. Renal function monitoring

Part of rivaroxaban is excreted through the kidneys. When renal function declines, drug accumulation increases the risk of bleeding. Creatinine and creatinine clearance rate should be regularly monitored.

3. Adjustment of renal insufficiency

Patients with decreased creatinine clearance may need to adjust the dosage or avoid use, and should follow the guidance of a doctor. Do not increase or decrease the dosage on your own.

4. Attention for elderly patients

Elderly people often experience a physiological decline in kidney function, and even if creatinine is normal, creatinine clearance rate should be evaluated to prevent drug accumulation.

5. Impact of combined diseases

6. Monitoring frequency

2. Other anticoagulant drugs

3. Strong CYP3A4 and P-gp dual inhibitors

Ketoconazole, itraconazole, ritonavir, etc. can increase the blood concentration of rivaroxaban, increase the risk of bleeding, and should usually be avoided in combination.

4. Strong dual inducer of CYP3A4 and P-gp

Rifampicin, carbamazepine, phenytoin, etc. can reduce the blood concentration of rivaroxaban, which may weaken the anticoagulant effect, and should be avoided in combination.

5. Antidepressants and antiepileptic drugs

Partial selective serotonin reuptake inhibitors and antiepileptic drugs may affect coagulation or platelet function, and caution should be exercised when used in combination.

6. Herbs and Health Products

St. John's wort can induce hepatic enzymes and reduce the efficacy of rivaroxaban. Consult a doctor or pharmacist before taking.
