Bedaquiline is a diarylquinoline antituberculosis drug that exerts bactericidal activity by inhibiting Mycobacterium tuberculosis ATP synthase, and is mainly used for the treatment of multidrug-resistant tuberculosis (MDR-TB).

I. Dosage and Administration

1. Standard adult regimen

The recommended dose is 400 mg once daily orally for weeks 1 to 2; 200 mg three times weekly (at least 48 hours apart) orally for weeks 3 to 24. The total course is 24 weeks.

2. Administration method

It should be taken with food to improve bioavailability. Swallow the tablet whole; do not chew or crush.

3. Combination therapy

Bedaquiline must not be used alone and must be combined with other antituberculosis drugs as part of a multidrug-resistant tuberculosis combination regimen.

4. Management of missed doses

If a dose is missed on a scheduled dosing day, take it as soon as possible and then continue the original regimen. If the next dose is near, skip the missed dose; do not double the dose.

II. Efficacy

1. Mechanism of action

Bedaquiline exerts bactericidal activity by inhibiting Mycobacterium tuberculosis ATP synthase and blocking bacterial energy metabolism. It is active against both susceptible and multidrug-resistant tuberculosis strains.

2. Clinical positioning

It is mainly used in combination therapy for multidrug-resistant tuberculosis, and can shorten the course and improve cure rates. It is not recommended for first-line treatment of drug-susceptible tuberculosis.

3. Efficacy characteristics

When combined with an optimized background regimen, it can accelerate sputum culture conversion and improve treatment outcomes. Efficacy is closely related to the completeness of the combination regimen and patient adherence.

III. Precautions

1. Cardiac toxicity

It can cause QT interval prolongation and increase the risk of arrhythmia. Electrocardiography should be monitored before and during treatment. Avoid concomitant use with other drugs that prolong the QT interval.

2. Hepatotoxicity

It may cause elevated liver enzymes. Liver function should be monitored before and during treatment. If signs of liver injury occur, discontinue the drug and evaluate.

3. Drug interactions

Bedaquiline is metabolized by CYP3A4. Strong CYP3A4 inducers (such as rifampin) can reduce its blood concentration and should be avoided; strong CYP3A4 inhibitors (such as ketoconazole) can increase its blood concentration and should be used with caution.

4. Special populations

Use with caution in patients with severe hepatic impairment. Data on use during pregnancy and lactation are limited, and the benefits and risks should be weighed. The safety and effectiveness of pediatric use have not been fully established.

5. Adherence

The regimen must be followed strictly; do not stop or reduce the dose on your own. Missed doses or interrupted treatment may lead to treatment failure or drug resistance.

6. Monitoring requirements

During treatment, electrocardiography, liver function, sputum bacteriology, and clinical response should be monitored regularly, and adverse reactions should be managed promptly.
