Trelegy Ellipta is a triple compound inhalation powder that contains three different mechanisms of action: inhaled corticosteroids, long-acting anticholinergic drugs, and long-acting beta agonists. It is used for long-term maintenance treatment of chronic obstructive pulmonary disease and asthma.

1、 Drug name

Fluticagrelor inhalation powder (English name: Trelegy Ellipta).

2、 Indications

Long term, once daily maintenance therapy for chronic obstructive pulmonary disease (COPD), including patients with chronic bronchitis and/or emphysema, suitable for the following two groups of people:

Patients who are already using a fixed dose combination of fluticasone/vilantoro but still need further improvement in airflow limitation;

Patients who are already using a fixed dose combination of Umetamide Bromide and Fluticasone/Valantro.

Important limitation: Not applicable for the relief of acute bronchospasm or the treatment of asthma.

3、 Specifications and characteristics

Dosage form: Inhalation powder, composed of light gray and beige plastic inhaler, containing 2 aluminum foil blister strips.

A blister containing fluticasone (100mcg/blister) and lactose monohydrate.

The other blister contains Umetam Bromide (62.5mcg/blister), Valantro (25mcg/blister), magnesium stearate, and lactose monohydrate (containing milk protein).

4、 Main components

Each dose contains three active ingredients:

Fluticasone: 100mcg (inhaled corticosteroid).

Umetamine bromide: 62.5mcg (anticholinergic drug).

Vilantinol: 25mcg (long-acting β 2-adrenergic receptor agonist, LABA)。

5、 Usage and dosage

Usage: Administer only by oral inhalation.

Dosage: Once a day, inhale 1 dose each time.

Post medication treatment: Rinse mouth with water immediately after inhalation (do not swallow) to reduce the risk of oral and pharyngeal candidiasis infection.

6、 Dose adjustment

Elderly patients, patients with renal impairment, or patients with moderate liver impairment do not need to adjust the dosage.

7、 Medication precautions

Before/after meals: If there is no specific requirement, it is recommended to use it at the same time every day.

Missed medication: If medication is forgotten, it should be taken as soon as possible, but not more than once a day.

Overdose treatment: Stop medication immediately and receive symptomatic supportive treatment.

Other precautions:

Not suitable for relieving acute symptoms.

Do not use in combination with other drugs containing LABA.

If abnormal bronchial spasms or allergic reactions occur, stop taking the medication immediately and seek medical attention.

8、 Medication for special populations

Pregnant women: There is not enough data to confirm safety, and it is necessary to weigh the pros and cons before use.

Breastfeeding women: Unknown if it is secreted through breast milk, use with caution.

Children: Safety and efficacy not established, not applicable.

Patients with liver function impairment: Patients with moderate to severe liver disease need to be closely monitored for systemic corticosteroid related side effects.

Individuals with renal impairment: No need to adjust dosage.

9、 Adverse reactions

The most common adverse reactions (incidence ≥ 1%) include:

Headache, back pain, taste disorders, diarrhea, cough, oropharyngeal pain, and gastroenteritis.

10、 Contraindications

Prohibited for individuals with severe allergies to milk protein;

It is contraindicated for individuals who are allergic to fluticasone, sunitinib bromide, Valantro, or any excipients.

11、 Drug interactions

Strong CYP3A4 inhibitors (such as ketoconazole): use with caution, may increase the risk of systemic corticosteroids and cardiovascular side effects.

Monoamine oxidase inhibitors and tricyclic antidepressants: may enhance the effects of vilantoro on the vascular system, requiring extreme caution.

β - receptor blockers: may block bronchiectasis and cause severe bronchospasm.

Anticholinergic drugs: avoid combination with other drugs containing anticholinergic ingredients.

12、 Storage method

Store at room temperature (20 ° C-25 ° C), avoiding direct heat sources and sunlight.

Unopened medication boxes should be placed in moisture-proof aluminum foil trays and used up within 6 weeks after opening.
