Trelegy Ellipta inhalation powder is a triple inhalation formulation containing fluticasone furoate, remdesivir bromide, and vilantol, used for the maintenance treatment of chronic obstructive pulmonary disease (COPD) and asthma. The indications are clear, and medication should pay attention to inhalation methods, adverse reaction monitoring, and medication for special populations.

1. Indications

(1) Chronic obstructive pulmonary disease (COPD)

Long term maintenance treatment for COPD patients, including chronic bronchitis and emphysema, can reduce acute exacerbations and improve lung function.

(2) Asthma

Used for maintenance treatment of asthma patients aged 18 and above, it should be used in combination with other asthma control drugs and is not suitable for relieving acute bronchospasm.

2. Medication precautions

(1) Correct inhalation method

Before use, it is necessary to read the instruction manual and master the operating skills of the Ellipta inhaler. Rinsing mouth after inhalation can reduce the risk of oral candidiasis infection.

(2) Adverse reaction monitoring

Common adverse reactions include headache, throat irritation, and cough. Be alert to the occurrence of serious adverse reactions such as pneumonia and cardiovascular events.

(3) Contraindications

Prohibited for individuals allergic to milk protein or formulation ingredients. Do not use as emergency medication during severe asthma attacks.

(4) Medication for special populations

During pregnancy, weigh the pros and cons, and use with caution during lactation. Patients with liver dysfunction do not need to adjust the dosage, while those with severe renal dysfunction should be cautious.

The combination with strong CYP3A4 inhibitors may increase systemic exposure to fluticasone furoate, and adrenal suppression symptoms need to be monitored.
