The standard dose of Ponatinib is 45mg orally once daily, and the specific number of tablets to be taken needs to be calculated according to the drug specifications. For a common 15mg tablet specification, 3 tablets should be taken daily.

1 Standard Dose Protocol

(1) Chronic myeloid leukemia in chronic phase: The initial dose is 45mg per day, which can be reduced to 15mg per day after disease control.

(2) Accelerated/rapid change period: Continue daily at 45mg until disease progression or intolerable toxicity occurs.

2 Dose Adjustment Rules

(1) Arterial occlusion: If relevant symptoms occur, the medication should be stopped immediately and reduced to 15mg/day after recovery.

(2) Hepatotoxicity: Elevated levels of 3-4 transaminase require temporary suspension of administration, and a reduction of 15mg/day is required after recovery.

(3) Pancreatitis: When lipase is elevated and accompanied by symptoms, pause and reduce the dosage after recovery.

3. Medication for special populations

(1) Liver dysfunction: Child Pugh A/B grade should be reduced to 30mg/day, and C grade should be avoided.

(2) Elderly patients: It is recommended to start treatment with a lower dose of 30mg/day.

4 Medication precautions

(1) Whole piece ingestion: Do not break or chew, can be taken with food.

(2) Omission treatment: If it exceeds 12 hours on the same day, no more medication will be taken, and normal medication will be taken the next day.
