Gilteritinib is an FLT3 tyrosine kinase inhibitor primarily used to treat relapsed or refractory acute myeloid leukemia (AML) with FLT3 mutation positivity. Its efficacy is significant and well tolerated, providing precise treatment options for patients with specific gene mutations.

1. Indications

(1) FLT3 mutation positive AML: Girotinib is suitable for adult patients with relapsed or refractory acute myeloid leukemia carrying FLT3-ITD or FLT3-TKD mutations, exerting anti-tumor effects by targeting mutant proteins.

2. Therapeutic effect

(1) Response rate: Clinical trials have shown that the complete response rate (CR) of gefitinib monotherapy is 21%, and the composite complete response rate (CRc) is 54%, significantly better than traditional chemotherapy.

(2) Survival benefit: The median overall survival (OS) was 9.3 months, which was 3.7 months longer than the control group, and adverse reactions were controllable.

3. Medication plan

(1) Standard dose: Take 120mg orally once daily, continue medication until disease progression or intolerable toxicity occurs.

(2) Dose adjustment: Depending on the severity of adverse reactions, the dosage can be temporarily reduced to 80mg/day. If it is still intolerable, the medication needs to be discontinued.

4. Special populations

(1) Liver dysfunction: Mild injury does not require dose adjustment, moderate to severe patients should use with caution and closely monitor.

(2) Elderly patients: The therapeutic effect of patients aged 65 and above is similar to that of young patients, but more attention should be paid to adverse reactions.
