Dabrafenib is a BRAFV600 mutation inhibitor mainly used to treat malignant tumors such as BRAFV600E/K mutation positive melanoma and non-small cell lung cancer. The standard dosage is adjusted according to the indications and the patient's age and weight. It needs to be swallowed whole and the treatment scope covers various solid tumors and hematological tumors.

1. Standard usage and dosage

(1) Adult melanoma: Take 150mg orally twice a day (with a 12 hour interval) on an empty stomach or with meals.

(2) Non small cell lung cancer: 150mg orally per day, in combination with trametinib, until disease progression or intolerable toxicity.

(3) Pediatric patients: Adjust the dosage according to body weight, with a daily dose of 150mg for patients with a body surface area of ≥ 1.2 square meters and 75mg for patients with a body surface area of<1.2 square meters.

2. Principle of dose adjustment

(1) Fever ≥ 38.5 ℃: Suspend medication until recovery, and reduce the dosage level for subsequent treatment.

(2) Grade 3 skin reaction: Suspend administration until it returns to ≤ Grade 1, then restart at the original dose or at a reduced dose.

(3) Grade 4 adverse reactions or intolerable toxicity: permanent discontinuation of medication.

3. Scope of treatment

(1) FDA approved indications: BRAFV600E mutation positive unresectable/metastatic melanoma, non-small cell lung cancer, thyroid cancer, etc.

(2) Approved indications in China include adjuvant therapy for melanoma and indications for solid tumors, which have been included in the medical insurance catalog.

(3) Medication restriction: Only applicable to BRAFV600 mutation positive patients, confirmed by genetic testing.

4. Special precautions

(1) Omission treatment: If it is ≥ 6 hours before the next dose, it can be taken again, otherwise it will be skipped.

(2) Drug interactions: Avoid combination with strong CYP3A4/CYP2C8 inducers.

(3) Treatment cycle: Continue medication until disease progression or intolerable toxicity occurs.
