As a targeted therapy drug, Avapritinib is mainly contraindicated for patients with special physiological conditions and severe organ dysfunction. During medication, specific foods and drug interactions must be strictly avoided. Additionally, attention should be paid to the restrictive conditions during treatment to ensure medication safety.

1. Prohibited groups

It is contraindicated for women during pregnancy. Animal studies have shown an embryotoxic risk. Before treatment, the pregnancy status should be confirmed and effective contraceptive measures should be taken.

It is prohibited for lactating women to use this medication, as it may be secreted through breast milk. Breastfeeding is also prohibited during the treatment period and for one week after discontinuation of the medication.

Patients with severe liver dysfunction (Child-Pugh Class C) are prohibited from using this medication, as it may exacerbate drug accumulation and lead to toxic reactions.

Patients allergic to afatinib or its excipients are prohibited from using this medication, as it may trigger severe allergic reactions, including anaphylactic shock.

2. Taboo foods

Grapefruit and its products are prohibited for consumption due to the presence of furanocoumarins, which can inhibit CYP3A4 enzymes and significantly increase plasma drug concentration.

St. John's wort products should be avoided as they may induce CYP3A4 enzyme activity, leading to reduced efficacy.

High-fat diet should be restricted as it may delay drug absorption. It is recommended to take the medication on an empty stomach or 2 hours after a meal.

3. Medication restrictions

(1) Pre-treatment assessment

Baseline liver function values need to be tested, and medication should be temporarily suspended if ALT/AST is greater than 3 times the upper limit of normal.

Risk factors for QT interval prolongation, including electrolyte abnormalities or a history of heart disease, need to be screened.

(2) Monitoring during treatment

Monitor cognitive function monthly, and discontinue the medication immediately if hallucinations or confusion occur.

Regularly undergo skin examinations to be vigilant against the occurrence of drug-related photosensitive dermatitis.

(3) Restrictions for special groups

Children under 18 years of age are prohibited from using this product, as its safety and effectiveness have not been established.

Medication administration for elderly patients should be cautious, and the dosage may need to be adjusted to reduce the risk of adverse reactions.
