The dosage of precipitated calcium carbonate tablets (Caltan) for special populations should be adjusted or prohibited according to physiological and pathological conditions. The following provides detailed instructions on medication precautions for six groups of people, including pregnant and lactating individuals.

Pregnant women

(1) The standard calcium supplement dosage is 1000-1200mg per day, taken in divided doses.

(2) Blood calcium levels need to be monitored to avoid hypercalcemia caused by exceeding 2.6 mmol/L.

(3) Patients with combined pregnancy induced hypertension should be cautious as calcium supplements may affect the effectiveness of antihypertensive drugs.

2 lactating women

(4) The recommended dosage should not exceed 1500mg per day, as breastfeeding can accelerate calcium loss.

(5) Long term high-dose use may lead to excessive calcium concentration in breast milk, causing constipation in infants.

(6) It is recommended to use it in combination with vitamin D, but the daily intake of vitamin D should be controlled to be ≤ 400IU.

Three medications for children

(7) Under 6 years old, take 20mg/kg/day in 3 doses and use a suspension form.

(8) The daily total dose for children under 12 years old should not exceed 1000mg, and it can increase to 1200mg during adolescence.

(9) Premature infants should receive calcium supplementation under the supervision of NICU, and caution should be taken against renal calcium deposition.

4 elderly patients

(10) It is recommended to take a daily dose of no more than 1200mg, divided into 3-4 times with meals.

(11) Patients with combined osteoporosis need to use bisphosphonates in combination, and the effect of calcium alone is limited.

(12) Regularly monitor creatinine clearance rate and reduce by 50% when GFR<30ml/min.

5 renal insufficiency

(13) Mild to moderate patients (eGFR30-89) need to limit their intake to below 800mg per day.

(14) Severe patients (eGFR<30) are prohibited from using it to avoid aggravating vascular calcification.

(15) Dialysis patients should receive medication after dialysis to prevent excessive calcium phosphorus product.

6 Other special circumstances

(16) Prohibited for individuals with hyperparathyroidism, which may exacerbate hypercalcitonin crisis.

(17) Patients with granulomatous disease should be alert to increased sensitivity to vitamin D.

(18) Long term users of antacids should pay attention to the impact of gastric acid deficiency on calcium absorption.
