Co Diovan is a compound antihypertensive drug composed of valsartan and hydrochlorothiazide, suitable for primary hypertension with poorly controlled single component. The following treatment guidelines are provided in terms of indications, usage and dosage, precautions, etc.

1. Indications and mechanisms of action

Fudaiwen is suitable for patients with primary hypertension who have insufficient efficacy with valsartan or hydrochlorothiazide alone. Valsartan is an angiotensin II receptor antagonist that lowers blood pressure by blocking the RAAS system; Hydrochlorothiazide is a thiazide diuretic, which can reduce sodium water retention, and both of them can synergistically enhance the antihypertensive effect.

2 Usage and Dosage

(1) Initial dose: It is recommended to take one tablet per day (containing 80mg valsartan and 12.5mg hydrochlorothiazide) orally on an empty stomach or after a meal.

(2) Dose adjustment: Depending on blood pressure control, it can be increased to 1 tablet (160mg/12.5mg) or 2 tablets (160mg/25mg) per day, with a maximum dose not exceeding 320mg/25mg per day.

3 Precautions

(1) Contraindications: Allergies to sulfonamide drugs, severe liver and kidney dysfunction, and contraindications during pregnancy and lactation.

(2) Electrolyte monitoring: Regularly check the levels of potassium, sodium, magnesium, and uric acid in the blood, especially for those with renal insufficiency or those taking diuretics in combination.

(3) Hypotension risk: Dehydrated patients should correct their volume before administering medication, and caution should be exercised when starting the dosage for elderly and heart failure patients.

4 Adverse reactions

Common symptoms include dizziness, hypotension, hyperkalemia (related to valsartan), hypokalemia, and hyperuricemia (related to hydrochlorothiazide). Rare vascular and neurological edema requires immediate discontinuation of medication and medical attention.

5 Drug Interactions

(1) Non steroidal anti-inflammatory drugs: weaken the antihypertensive effect and increase the risk of kidney damage.

(2) Lithium salt: Hydrochlorothiazide increases blood lithium concentration and toxicity needs to be monitored.

(3) Strong CYP450 inhibitors: may increase exposure to valsartan, blood pressure changes need to be observed.
