Precautions for taking compound therapy: electrolyte imbalance, hypotension, and renal function monitoring

Co Diovan, as a combination of valsartan and hydrochlorothiazide, requires special attention to the three core risks of electrolyte imbalance, orthostatic hypotension, and renal function monitoring. The following will explain the key points of clinical monitoring and intervention measures.

1. Electrolyte disorder management

(1) Monitoring frequency: Within the first week of treatment, blood potassium, sodium, magnesium, and uric acid levels need to be tested, followed by regular follow-up every 3-6 months.

(2) High risk populations: elderly people, individuals with renal insufficiency, and patients taking diuretics should shorten the monitoring interval to 2-4 weeks.

(3) Intervention threshold: For blood potassium levels below 3.5mmol/L, potassium supplementation is required. For levels above 5.5mmol/L, medication should be reduced or discontinued, following the electrolyte regulation protocol.

2. Prevention of Hypotension

(1) Medication timing: It is recommended to take the first dose before bedtime to avoid the risk of falls caused by orthostatic hypotension.

(2) Capacity assessment: Correct dehydration status before treatment, and increase water intake (1.5-2L per day) during summer or diarrhea.

(3) Danger signal: When systolic blood pressure is less than 90mmHg and accompanied by dizziness, immediately lie down and seek medical attention to adjust the dosage.

3. Renal function monitoring

(1) Baseline assessment: Blood creatinine/eGFR must be checked before medication, and hydrochlorothiazide is contraindicated for severe renal insufficiency (eGFR<30).

(2) Dynamic observation: If eGFR decreases by more than 30% within one month of medication, the medication should be stopped to investigate secondary factors such as renal artery stenosis.

(3) Special warning: Combination use of NSAIDs may cause acute kidney injury, and it is necessary to strengthen monitoring of urine output and kidney function.

4 Drug Interactions

(1) Potassium elevation risk: Avoid using ARBs, potassium sparing diuretics, or CYP3A inhibitors (such as clarithromycin) in combination to prevent hyperkalemia.

(2) Hypotension synergy: When used in combination with calcium antagonists, caution should be taken against superimposed hypotension, and staggered administration is recommended.

(3) Toxicity of lithium salts: Hydrochlorothiazide reduces lithium excretion, and combined use requires monitoring of blood lithium concentration and adjusting dosage.
