Precautions for taking Olaparib: Bone marrow suppression, interstitial lung disease, and drug interactions

The safety management of Olaparib requires regular blood routine monitoring, vigilance for new respiratory symptoms, and strict screening for concomitant medication. If there is grade 3 or above bone marrow suppression or suspected ILD, the dosage should be reduced or stopped in a timely manner, and multidisciplinary management should be combined.

1、 Bone marrow suppression

1.1 Characteristics of occurrence

Olaparib can cause thrombocytopenia, neutropenia, and anemia, often occurring 2-4 weeks after medication and exhibiting dose-dependent toxicity.

1.2 Monitoring Requirements

Complete blood count (CBC) should be rechecked before treatment and monthly.

If there is fever, bleeding, bruising, or worsening fatigue, a blood routine should be immediately rechecked.

1.3 Principles of Handling

Level 1-2: Continue medication and strengthen monitoring.

≥ Level 3: Suspend medication until it returns to ≤ Level 1, then restart at the original dose or reduce by 100mg (twice daily).

If recurrence persists after reduction, the dosage can be further reduced to 200mg (twice daily).

2、 Interstitial lung disease (ILD)

2.1 Risk Warning

The incidence rate is about 1% to 2%, but it can rapidly progress to respiratory failure and requires high attention.

2.2 Monitoring and Identification

Newly developed or aggravated dry cough, dyspnea, fever, and hypoxemia require immediate chest high-resolution CT (HRCT) evaluation.

2.3 Handling Measures

Diagnosed or highly suspected ILD: permanently discontinue medication and administer glucocorticoid therapy.

After excluding ILD, restarting Olaparil can be considered, but the benefits and risks need to be carefully evaluated.

3、 Drug interactions

3.1 Medications that should be avoided in combination

Strong CYP3A4 inhibitors (such as itraconazole, clarithromycin, ritonavir): can significantly increase the blood concentration of olaparib and increase the risk of toxicity.

Strong CYP3A4 inducers (such as rifampicin, carbamazepine, St. John's wort): can reduce the efficacy of olaparib.

3.2 Caution should be exercised when using drugs in combination

Statins, lipid-lowering drugs, warfarin, and other drugs metabolized by CYP3A4 need to be monitored for blood drug concentration or INR when used in combination.

Combination radiotherapy and chemotherapy may exacerbate bone marrow suppression and require individualized evaluation.

3.3 Traditional Chinese Medicine and Health Products

Avoid using St. John's wort, ginseng, and ginkgo leaf preparations as they may affect CYP3A4 activity.
