
Finelinone is a selective nonsteroidal mineralocorticoid receptor antagonist, which is mainly used to treat chronic kidney disease associated with type 2 diabetes. The standard dose is 10mg or 20mg orally once daily, and the specific dose needs to be adjusted according to the patient's blood potassium level and renal function.
1 Standard Dose Protocol
(1) Initial dose: The recommended initial dose is 10mg or 20mg, taken orally once daily, with or without food.
(2) Maintenance dose: Adjust the dose according to blood potassium levels and renal function, with a maximum recommended dose of 20mg/day.
Principle of dose adjustment
(1) Blood potassium>5.5mmol/L: Suspend administration until blood potassium is ≤ 5.0mmol/L, then reduce to 10mg/day or switch to other treatments.
(2) EGFR<25mL/min/1.73m ²: Initial treatment is not recommended. If eGFR continues to decrease during treatment, risk benefit assessment should be conducted.
3. Medication for special populations
(1) Liver dysfunction: mild to moderate injury does not require adjustment; Severe injury (Child Pugh C) lacks data, use with caution.
(2) Elderly individuals (≥ 65 years old): No dose adjustment is required, but close monitoring of kidney function and blood potassium is necessary.
4. Handling of missed services
(1) If the missed dose time is ≤ 12 hours, the current dose should be taken immediately.
(2) If the missed dose exceeds 12 hours: skip the current dose and take the medication according to the original plan the next day. Double dose supplementation is prohibited.
