Finerenone is a non steroidal mineralocorticoid receptor antagonist used to treat chronic kidney disease and type 2 diabetes. The following will provide a detailed introduction to its medication guidelines, including medication timing, dosage adjustment, and management of missed doses.

1. Medication duration

Finerenone should be taken daily at a fixed time and may or may not be taken with food. Although food does not affect its overall absorption, taking it with meals may help reduce gastrointestinal discomfort.

2. Dose adjustment

(1) Patients with renal insufficiency

Mild to moderate renal impairment (eGFR25-60mL/min/1.73m ²) does not require dose adjustment. Not recommended for severe renal impairment (eGFR<25mL/min/1.73m ²).

(2) Patients with liver dysfunction

Mild liver dysfunction (Child Pugh A grade) does not require dose adjustment. Moderate to severe liver function impairment (Child Pugh B/C grade) should be avoided.

(3) Drug interactions

When used in combination with potent CYP3A4 inhibitors, the dosage should be halved. The combination with intermediate acting CYP3A4 inhibitors requires close monitoring of blood potassium levels.

3. Handling of missed services

If Finerenone is missed, it should be taken immediately upon recollection. If it is close to the next dose (<6 hours), skip the missed dose and take the next dose according to the original plan. Do not take double doses at the same time.

4. Precautions

Regular monitoring of blood potassium levels and renal function is required during treatment. If hyperkalemia occurs (blood potassium>5.5mEq/L), medication should be temporarily discontinued until blood potassium returns to normal, and then consider reducing dosage and restarting treatment.
