Olmetec, as a long-acting antihypertensive drug, has significant therapeutic effects and good safety. However, caution should be exercised when discontinuing the medication to avoid blood pressure rebound. Olmetec exerts its antihypertensive effect by selectively blocking angiotensin II receptors, and its efficacy needs to be evaluated based on individual patient conditions. The following provides a detailed explanation from three aspects: evaluation of treatment efficacy, precautions for discontinuation, and effectiveness, in order to provide guidance for clinical medication.

1. Effect of action

(1) Pharmacological mechanism: Specific antagonism of AT1 receptor, inhibition of vascular constriction and aldosterone secretion, and sustained antihypertensive effect for more than 24 hours.

(2) Effective time: The peak blood pressure is reached 1-2 hours after oral administration, and the steady-state antihypertensive effect is achieved within 2 weeks. The most significant therapeutic effect is observed between 4-8 weeks.

(3) Blood pressure reduction amplitude: Single drug treatment can reduce systolic blood pressure by an average of 10-15 mmHg and diastolic blood pressure by 6-8 mmHg.

2. Evaluation of treatment efficacy

(1) Hypotensive effect: Monotherapy can achieve blood pressure control in approximately 50% -60% of hypertensive patients (<140>50%).

(2) Target organ protection: it can significantly reduce the risk of left ventricular hypertrophy, delay the progress of diabetes nephropathy, and reduce proteinuria by about 35%.

(3) Special population efficacy: The blood pressure reduction in elderly patients is comparable to that in young patients, but the dosage needs to be adjusted more slowly to avoid orthostatic hypotension.

3. Precautions for discontinuation of medication

(1) Gradual reduction: Sudden discontinuation of medication may cause blood pressure rebound. It is recommended to reduce dosage by 50% every 2-4 weeks (e.g. 40mg → 20mg → 10mg).

(2) Alternative therapy: If dressing change is required, substitute medication should be gradually added while reducing dosage, with overlapping use for 1-2 weeks to ensure a smooth transition.

(3) Monitoring requirements: Monitor blood pressure for at least 2 weeks after discontinuing medication, with a focus on morning peak blood pressure and nighttime blood pressure fluctuations.

(4) High risk patients: Patients with concomitant coronary heart disease and heart failure who discontinue medication need to be hospitalized for monitoring to avoid sudden increases in blood pressure that may trigger cardiovascular events.
