Olmetec is not suitable for all hypertensive patients. The following three groups of people should be prohibited or avoided from using it, and special attention should be paid.

1、 Pregnant women

Reason for prohibition: Omeprazole can act on the renin-angiotensin system, and its use in mid to late pregnancy can lead to fetal kidney damage, oligohydramnios, skull dysplasia, and even fetal death.

Early pregnancy: Although the risk is relatively low, once pregnancy is detected, medication should be stopped immediately and medical attention sought.

Women of childbearing age: Effective contraceptive measures should be taken during medication.

Pregnant women planning to conceive should inform their doctors in advance and switch to a safe blood pressure reduction plan during pregnancy.

Breastfeeding period: It is not yet clear whether milk is secreted, and it is recommended to avoid or suspend breastfeeding.

2、 Severe liver dysfunction patients

Reason for prohibition: Olmesartan is mainly metabolized in the liver. In patients with severe liver dysfunction, drug clearance slows down, blood drug concentration increases, and the risk of adverse reactions increases.

Symptoms: Possible occurrence of hypotension, dizziness, and worsening fatigue.

Mild to moderate liver dysfunction: should be used with caution, the initial dose should be reduced, and closely monitored.

Monitoring items: liver function, blood pressure, and adverse reactions.

Cholestasis: It can affect drug absorption and metabolism, and requires special attention.

3、 Patients with cholestasis

Reason for prohibition: Cholestasis can significantly affect the absorption and excretion of olmesartan, leading to drug accumulation.

Risks: Hypotension, deterioration of renal function, and electrolyte imbalance.

Treatment: Avoid using and choose other antihypertensive drugs that do not rely on bile excretion.

For patients with combined biliary diseases: biliary function should be evaluated before medication.
