Meloxicam needs to be adjusted in dosage or discontinued according to physiological conditions in special populations, including those with pregnancy, lactation, liver and kidney dysfunction, etc. The following sub items provide specific indications and medication precautions for special populations to ensure treatment safety and effectiveness.
1. Osteoarthritis
(1) Relieve symptoms
Suitable for osteoarthritis of large joints such as knees and hips, it can significantly improve joint pain and morning stiffness. The recommended dosage is 7.5-15mg/day, and the efficacy should be evaluated by continuous use for 2 weeks.
2 Rheumatoid arthritis
(2) Combination therapy
As an adjuvant therapy with anti rheumatic drugs (DMARDs) to improve the condition, it can effectively reduce joint swelling and tenderness. Regular monitoring of blood routine and liver and kidney function is required during the treatment period.
Ankylosing spondylitis
(3) Improve activity level
It has a clear therapeutic effect on axial spondyloarthritis and can improve the patient's spinal mobility. Nighttime administration is more beneficial for controlling morning stiffness symptoms.
4 Other indications
(4) Short term analgesia
It can be used for short-term symptom control such as acute gout attacks and postoperative pain, but the course of treatment should not exceed 5 days. Please note that this medication is only for symptomatic treatment of gout.
Medication during pregnancy
(5) The use of meloxicam during late pregnancy may inhibit prostaglandin synthesis, leading to premature closure of fetal ductus arteriosus or uterine atony, and increasing the risk of prolonged labor. Medication should be absolutely discontinued after 30 weeks of pregnancy.
6 Medications for lactation
(6) Suspending breastfeeding medication can result in the secretion of breast milk, and breastfeeding infants may experience renal dysfunction. It is recommended to suspend breastfeeding during the treatment period and within 5 days after discontinuing medication, and switch to formula milk powder as a substitute for feeding.
7 Medications for Children
(7) Prohibited for children under 2 years old, limited to short-term use (≤ 7 days) for patients aged 2-18. The dosage should be adjusted according to body weight, with a maximum daily dose not exceeding 0.25mg/kg.
8 Elderly medication
(8) The starting dose for patients over 65 years old should be reduced by 50%, especially for monitoring renal function. The maximum daily dose should not exceed 7.5mg to avoid accumulation and gastrointestinal bleeding.
9 liver dysfunction
(9) Moderate to severe use of Child Pugh B/C grade patients is prohibited, while mild impairment (Child Pugh A) requires a reduction to 7.5mg/day and weekly monitoring of transaminase levels.
10 renal insufficiency
(10) Disable GFR<30ml/min according to GFR adjustment; Administer 30-60ml/min every other day. Dialysis patients should increase their dosage by 50% after dialysis to avoid excessive drug clearance.
