The contraindicated population of Capasertib mainly includes special populations such as patients with type 1 diabetes and patients with severe liver and kidney dysfunction. It is necessary to strictly assess the risk benefit ratio and take corresponding management measures.

Patients with type 11 diabetes

(1) Absolute taboo

Capasetinib may induce severe hyperglycemia. The risk of ketoacidosis in type 1 diabetes patients is significantly increased due to the absolute lack of insulin.

(2) Monitoring requirements

The type of diabetes must be screened before use. During treatment, blood glucose should be monitored daily and quick acting insulin should be prepared.

2. Severe liver dysfunction

(1) Child Pugh C grade

Safety data have not yet been established in patients with severe liver dysfunction, as drugs are mainly metabolized through the liver and may significantly increase exposure levels.

(2) Dose adjustment

Patients with moderate liver function impairment should reduce their dosage by 25% -50%, and avoid using bilirubin levels greater than 3 times the ULN.

3. Severe renal insufficiency

(1)eGFR<30mL/min

Patients with end-stage renal disease are prohibited from using it as it may worsen electrolyte imbalance and metabolic acidosis.

(2) Dialysis effects

Hemodialysis cannot effectively remove capecitabine, which may cause drug accumulation toxicity.

4 Other taboo groups

(1) Pregnancy period

Due to embryonic fetal toxicity, women of childbearing age need to use highly effective contraception during treatment and within one month after discontinuation of medication.

(2) Allergy history

Prohibited for individuals allergic to active ingredients or any excipients, as serious reactions such as Stevens Johnson syndrome may occur.
