There are clear contraindications and drug interaction red lines for the use of Seratinib. The following situations are absolutely prohibited or require extremely cautious use, and comprehensive screening must be completed before treatment.

1、 Absolute taboo group

Severe liver dysfunction: Child Pugh C grade or total bilirubin>3 times the upper limit and any elevated transaminases are prohibited from initial use.

Severe renal insufficiency: End stage renal disease patients with estimated glomerular filtration rate (eGFR)<30mL/min/1.73m ² or requiring dialysis are contraindicated.

Known allergy history: those who have an immediate hypersensitivity reaction to Seratinib or its excipients (such as microcrystalline cellulose).

2、 The risk of using CYP3A4 potent inhibitors in combination (to be strictly avoided or adjusted)

Typical drugs: including clarithromycin, itraconazole, ketoconazole, ritonavir, and grapefruit/grapefruit products.

Core risk: Inhibiting the metabolism of Seratinib can increase its blood drug concentration several times, significantly increasing the risk of liver toxicity and QT interval prolongation.

Principle of treatment: Combination therapy is prohibited during the treatment period. If it is necessary to use, stop using Seratinib or switch to non interacting substitute drugs. Do not reduce the dosage and take it together on your own.

3、 Risk of using CYP3A4 potent inducer in combination (absolutely prohibited)

Typical drugs include rifampicin, phenytoin, carbamazepine, and St. John's wort (Hypericum perforatum).

Core risk: Accelerating the clearance of Seratinib, significantly reducing blood drug concentration, may lead to complete treatment failure.

Principle of treatment: Before medication, all concomitant medications and Chinese herbal medicines must be checked to confirm the absence of the aforementioned ingredients before treatment can begin.

4、 Other alert groups

Pregnancy: If there is evidence of fetal damage, women of childbearing age must use efficient contraception during treatment and within 6 months after discontinuing medication.

For those with previous pancreatitis or QTc interval>480ms: Consider using it only after evaluating the benefits outweigh the risks under close monitoring by a specialist.
