Acetazolamide is a carbonic anhydrase inhibitor commonly used to treat glaucoma, altitude sickness, edema, and certain types of seizures. Taking it correctly is important for both efficacy and tolerability.

1. Take on an empty stomach or with food?

It is generally recommended to take it with food to reduce gastrointestinal reactions such as stomach discomfort and nausea.

If your doctor specifically instructs you to take it on an empty stomach, follow that advice, usually 1 hour before or 2 hours after a meal.

Drink plenty of water while taking the medication to help with drug excretion and reduce the risk of kidney stones.

Avoid taking it with large amounts of calcium- or magnesium-containing antacids, as this may affect absorption.

2. What should I do if I miss a dose?

If you remember and there is still a long time until your next dose, take the missed dose as soon as possible.

If your next dose is approaching, skip the missed dose and take your next dose as originally scheduled.

Do not take a double dose at one time.

If you frequently miss doses, consider setting an alarm or using a pill organizer as a reminder.

3. Dose adjustment and precautions

The dosage varies by indication and must follow your doctor's instructions; do not increase or decrease it on your own.

Glaucoma: usually 250 mg per dose, 1 to 4 times daily.

Altitude sickness: often started 24 hours before ascent, 125 to 250 mg per dose, twice daily.

Adjunctive treatment for epilepsy: calculated by body weight and requires a doctor's guidance.

Patients with renal impairment need a reduced dose or should use it with caution because the drug is excreted by the kidneys.

Patients with cirrhosis, severe renal failure, hypokalemia, or hyponatremia should use it with caution or avoid it.

Long-term use requires regular monitoring of electrolytes, blood counts, and renal function.

Possible side effects include numbness in the hands and feet, drowsiness, fatigue, and altered taste; seek medical attention promptly if severe.

Dose adjustment is needed when used with lithium, salicylates, or other diuretics.

4. Friendly reminders

Avoid alcohol while taking the medication to prevent worsening drowsiness.

Be cautious when driving or operating machinery until you know how the medication affects you.

If you develop a severe rash, persistent headache, vision changes, or significantly reduced urine output, seek medical attention immediately.

Discontinuation should follow your doctor's instructions, especially for long-term users; do not stop suddenly.
