During acetazolamide use, close attention should be paid to three issues: hypokalemia, metabolic acidosis, and sulfonamide allergy. Timely identification and management are key to safe medication use.

1. Hypokalemia

Cause: This drug promotes potassium excretion, and long-term or high-dose use can easily lead to decreased blood potassium.

Manifestations: Fatigue, muscle cramps, palpitations, arrhythmias, and in severe cases, paralysis.

Prevention and management:

Check blood potassium regularly before and during treatment.

Increase intake of potassium-rich foods such as bananas, oranges, potatoes, and spinach.

Supplement potassium under a doctor's guidance when necessary.

Avoid concomitant use with other potassium-wasting diuretics or laxatives.

If palpitations or obvious fatigue occur, seek medical attention immediately.

2. Metabolic acidosis

Cause: This drug inhibits carbonic anhydrase, which can cause bicarbonate loss and lead to hyperchloremic metabolic acidosis.

Manifestations: Deepened and accelerated breathing, drowsiness, headache, nausea, and confusion.

Prevention and management:

Long-term users should regularly check blood gas analysis and electrolytes.

Patients with renal impairment, diabetes, or lung disease are at higher risk and should use it with caution.

If abnormal breathing or altered consciousness occurs, seek medical attention promptly.

Your doctor may adjust the dose or supplement sodium bicarbonate as needed.

Avoid long-term high-dose use on your own.

3. Sulfonamide allergy

Cause: This drug is a sulfonamide derivative, and cross-allergy may occur in people allergic to sulfonamides.

Manifestations: Rash, urticaria, itching, and in severe cases, Stevens-Johnson syndrome or toxic epidermal necrolysis.

Prevention and management:

Before use, tell your doctor whether you have a history of sulfonamide allergy.

People allergic to sulfonamides should avoid it or use it with caution.

If rash, mucosal ulcers, or fever occur, stop the medication immediately and seek medical attention.

Severe skin reactions require emergency management, and the medication must not be continued.

People who have had a severe sulfonamide allergy should avoid using it again.

4. Other precautions

Drink plenty of water during treatment to reduce the risk of kidney stones.

Avoid alcohol to prevent worsening drowsiness and electrolyte disturbances.

Regularly monitor blood counts, electrolytes, and renal function.

Do not stop the medication or adjust the dose on your own; all changes require a doctor's guidance.

If a severe rash, persistent headache, vision changes, or significantly reduced urine output occurs, seek medical attention immediately.
