Fumarate propofol tenofovir tablets (Vemlidy) are antiviral drugs used to treat chronic hepatitis B. When using them, attention should be paid to liver function monitoring, kidney function evaluation, drug interactions, and medication for special populations to ensure efficacy and reduce the risk of adverse reactions.

1. Liver function monitoring

Regular monitoring of liver function is necessary during treatment, especially for patients with concurrent liver dysfunction or active hepatitis. If liver function deteriorates or acute hepatitis occurs, the treatment plan should be adjusted or the medication should be stopped in a timely manner.

2. Renal function assessment

Renal function should be evaluated before medication and during treatment. It is not recommended to use it for patients with creatinine clearance rates below 30mL/min. Long term treatment may lead to renal dysfunction, and regular monitoring of urinary protein and blood phosphorus levels is necessary.

3. Drug interactions

Avoid co administration with nephrotoxic drugs (such as aminoglycosides, vancomycin, etc.) as it may increase the risk of renal dysfunction. The combination with P-gp inducers (such as rifampicin) may reduce the blood concentration of Vemlidy, and the dosage needs to be adjusted.

4. Medication for special populations

(1) Pregnancy period

Pregnant women need to weigh the pros and cons before using it. Existing data does not show significant teratogenic risks, but caution is still needed.

(2) Lactation period

Breastfeeding women should avoid using drugs as they may be secreted through breast milk and have potential effects on the baby.

(3) Children

The safety of children under 12 years old and adolescents weighing less than 35kg has not been established and is not recommended for use.

5. Other precautions

During treatment, adverse reactions such as headache, abdominal pain, and fatigue may occur, which are usually mild and tolerable. If serious adverse reactions such as lactic acidosis or hepatic steatosis occur, the medication should be stopped immediately and medical attention should be sought.

6. Medication method

The recommended dosage is once daily, 25mg each time. The whole tablet should be swallowed and can be taken with food or on an empty stomach. If missed, it should be taken as soon as possible. If it is close to the next medication time, skip it and do not double the dosage.
