As a local NSAIDs preparation, Efrofen patch has targeted analgesic advantages in chronic musculoskeletal pain (such as osteoarthritis, lower back pain, and shoulder periarthritis). Its transdermal administration can reduce systemic adverse reactions, but it needs to be used in a standardized manner to balance efficacy and safety.

1. Application of osteoarthritis

(1) Target effect: By inhibiting local prostaglandin synthesis in joints, it alleviates pain and morning stiffness symptoms of osteoarthritis, especially suitable for superficial joints such as knee joints.

(2) Usage plan: Apply one patch directly to the painful joint daily for no more than 14 consecutive days, avoiding damaged skin.

(3) Therapeutic characteristics: Studies have shown that the pain relief rate for mild to moderate osteoarthritis is about 60-70%, which is 50% less than the gastrointestinal events caused by oral NSAIDs.

2. Lower back pain management

(1) Indication selection: More suitable for non radicular back pain (such as myofascial pain syndrome), with limited effect on radiating pain caused by lumbar disc herniation.

(2) Application technique: Apply longitudinally along the lumbar muscles, changing positions daily to avoid skin irritation, and combined with hot compress to enhance drug penetration.

(3) Course control: It is recommended to use for 7-10 days for acute back pain, and intermittent use for chronic pain to avoid long-term continuous use.

3. Treatment of shoulder periarthritis

(1) Medication positioning: The center of the patch covers the area with the most obvious tenderness in front and below the acromion, and during the active period, it can be combined with shoulder joint immobilization to enhance the efficacy.

(2) Combination therapy: Sequential use with physical therapy (such as ultrasound) can improve drug penetration, but with an interval of at least 30 minutes.

(3) Efficacy evaluation: It usually takes 3-5 days to take effect. If there is no improvement within 1 week, the diagnosis needs to be re evaluated. Be alert to organic lesions such as rotator cuff injury.

4. Special precautions

(1) Skin monitoring: The elderly and patients with diabetes need to strengthen skin observation, and the detection of irritation may be delayed due to hypoesthesia.

(2) Drug interactions: Avoid combination with oral NSAIDs and monitor bleeding tendency when used with anticoagulants.

(3) Taboo population: Asthma patients, history of NSAIDs allergy, and late pregnancy contraindications. During lactation, a doctor needs to evaluate the risk benefit ratio.
