When used in combination with valproic acid and chlorpromazine, significant reductions should be made and blood drug concentrations should be closely monitored; Appetite loss should be reinforced with nutritional support, while drowsiness can be improved by adjusting medication timing. 1、 Drug interactions with valproic acid and chlorpromazine

1.1 Combination with Valproic Acid

Sitriptylol can significantly increase the blood concentration of valproic acid (by about 30% to 50%), and when used in combination, the dose of valproic acid needs to be reduced by about 30%.

During the combination period, the blood concentration of valproic acid should be monitored to avoid drug accumulation leading to liver toxicity or hyperammonemia.

1.2 Combination with Chlorpromazine

Sitriptylol can increase the blood concentration of chlorpromazine and its active metabolite (N-demethylchlorpromazine) by about 2-3 times.

When used in combination, the initial dose of chlorpromazine should be halved and slowly titrated according to clinical response, while observing the worsening of sedation or drowsiness.

1.3 Management Suggestions

When combining the above drugs, the frequency of liver function and blood ammonia monitoring should be increased.

Before adding or discontinuing any medication, the risk of interaction needs to be reassessed.

2、 Treatment of decreased appetite

2.1 Dietary Adjustment

Eat small amounts and multiple meals, 5-6 meals per day, providing high protein, high calorie foods such as eggs, fish, and avocados.

Avoid greasy and strongly scented foods, and try cold or liquid foods (such as milkshakes, nutrient solutions) to improve tolerance.

2.2 Nutritional Support

If weight loss continues (>5% per month), oral nutritional supplements should be used, and tube feeding or short-term parenteral nutrition may be considered if necessary.
