1、 Severe liver dysfunction patients

Refers to Child Pugh C grade decompensated cirrhosis, or ALT/AST>5 × ULN with significantly elevated bilirubin levels.

1.2 Prohibition Basis

Sitriptylol is widely metabolized in the liver, and severe liver damage can lead to drug accumulation, increased liver toxicity, and central inhibitory risk.

1.3 Clinical recommendations

Absolutely prohibited. Patients of this type should not initiate treatment with sitagliptin.

Mild to moderate liver damage (Child Pugh A/B) patients should reduce their dosage and monitor liver function every 2-4 weeks.

2、 Severe renal insufficiency patients

2.1 Definition

Refers to end-stage renal disease with creatinine clearance rate (CrCl)<30mL/min or undergoing dialysis treatment.

2.2 Prohibition Basis

When renal function is severely impaired, the clearance of drugs and their metabolites slows down, increasing the risk of cumulative toxicity.

2.3 Clinical recommendations

Not recommended for use. If necessary, adjust the dosage under specialized guidance and monitor kidney function and electrolytes weekly.

3、 Individuals allergic to any ingredient of this product

3.1 Identification points

Individuals known to be allergic to sitagliptin or its excipients (such as lactose, starch, magnesium stearate, etc.).

3.2 Allergic Symptoms

May present with rash, urticaria, facial edema, difficulty breathing, or a history of anaphylactic shock.

3.3 Handling Principles

Absolutely prohibited and should be marked on the first page of the medical record.

If there is a suspected allergic reaction after medication, the medication should be stopped immediately and emergency treatment should be taken.
