Briviact is an antiepileptic drug mainly used for the treatment of partial epileptic seizures. Its medication for special populations needs to be adjusted according to pregnancy, lactation, liver and kidney function, and other conditions. The following will explain from two aspects: indications and medication for special populations.

1 Indications

1) Bu Li Xi Tan is suitable for adjuvant treatment of partial epileptic seizures (with or without secondary generalized seizures) in patients aged 4 years and above.

2) This drug exerts antiepileptic effects by selectively binding to synaptic vesicle protein SV2A, which can reduce the spread of abnormal discharges.

2. Medication for special populations

1) Pregnancy: Animal studies have shown that budesonide may cause fetal malformations, and pregnant women should fully evaluate the risks and benefits of using it. Effective contraceptive measures should be taken during the treatment period for women of childbearing age. If medication is necessary during pregnancy, close fetal monitoring should be conducted.

2) Breastfeeding period: Medications may be secreted through breast milk, and their potential effects on infants are not yet clear. It is recommended to suspend breastfeeding or switch to formula milk during treatment to avoid exposing newborns to drug risks.

3) Pediatric patients: The safety and efficacy of children aged 4 years and above have been established, and the dosage should be adjusted according to body weight. Insufficient medication data for children under 4 years old, not recommended for use.

4) Elderly patients: Due to possible liver and kidney dysfunction, it is recommended to start with low doses and closely monitor for adverse reactions. Elderly patients are more prone to neurological side effects such as drowsiness and dizziness.

5) Liver dysfunction: Patients with mild to moderate liver dysfunction usually do not need to adjust the dosage. Patients with severe liver function impairment (Child Pugh C grade) need to reduce their dosage by 50% and strengthen liver function monitoring.

6) Renal insufficiency: The recommended dose for patients with creatinine clearance rate of 30-50mL/min should be reduced by 25%, and for patients with creatinine clearance rate<30mL/min or dialysis, it should be reduced by 50%. Medication should be supplemented after hemodialysis.
