Drugs to Avoid When Using Eplerenone: CYP3A Inhibitors and Interactions

Inspra, as an aldosterone receptor antagonist, is mainly metabolized by the CYP3A enzyme system, and its combination with CYP3A inhibitors may lead to abnormally high blood drug concentrations. 1. Prohibited combination of potent CYP3A inhibitors

(1) Triazole antifungal drugs: Ketoconazole and itraconazole can increase the AUC of eplerenone by more than 5 times, and their combination is absolutely contraindicated.

(2) HIV protease inhibitors, such as ritonavir and nelfinavir, significantly increase the concentration of eplerenone by irreversibly inhibiting CYP3A.

(3) Macrolide antibiotics such as clarithromycin and gibberellin should be permanently avoided.

2. Moderate acting CYP3A inhibitors that require caution

(1) Calcium channel blocker: When combining diltiazem and verapamil, the dose of eplerenone should be adjusted to start at 25mg/day.

(2) Fluoroquinolones: Ciprofloxacin may moderately inhibit CYP3A. It is recommended to monitor blood pressure and blood potassium during combination use.

(1) Pre medication screening: Before prescribing eplerenone, a comprehensive evaluation of the patient's current medication history is required, with a focus on screening CYP3A inhibitors.

(2) Alternative: When antifungal treatment is necessary, fluconazole (a weak CYP3A inhibitor) can be used instead of itraconazole.

(3) Emergency treatment: If hypotension or hyperkalemia occurs after accidental combination therapy, the medication should be stopped immediately and intravenous fluid should be administered to correct it.
