Anamorelin works by stimulating the ghrelin receptor GHS-R1a.

1、 Severe liver dysfunction patients

Clear contraindications: Patients with severe liver dysfunction (such as Child Pugh C grade) are prohibited from using Anamorelin.

Main reason: The drug is mainly metabolized through the liver, and severe damage to liver function can lead to drug accumulation and increase the risk of adverse reactions.

Monitoring indicators: AST/ALT and bilirubin levels should be checked before and during medication. If there is a rapid deterioration of liver function, the medication should be stopped immediately.

2、 Severe renal insufficiency patients

Caution in use: Patients with end-stage renal disease or creatinine clearance rate below 30% of normal should use it with caution, usually as a relative contraindication.

Excretion effects: Severe renal dysfunction may affect the clearance of drug metabolites and increase exposure to toxicity.

Suggestion: If such patients need to use it, they should reduce the dosage under the guidance of a specialist and closely monitor renal function and electrolyte balance.

3、 Uncontrolled patients with type 2 diabetes

Taboo premise: If the patient has "uncontrolled" or "poorly controlled" diabetes (that is, continuous significantly high fasting glucose or frequent ketosis), it is generally not recommended to use Anamorelin.

Control sugar before medication: It is recommended to adjust insulin or take oral hypoglycemic drugs first, and evaluate the possibility of medication after blood sugar stabilizes (glycated hemoglobin<7.5%).

Joint monitoring: For diabetes patients who have used Anamorelin, if the blood sugar deteriorates rapidly, the drugs should be suspended and the hypoglycemic program should be strengthened.

4、 Other absolute taboos

Allergy history: Do not use this product for individuals who are allergic to any of its ingredients.

Endocrine tumors: Patients known to have growth hormone dependent tumors (such as active pituitary tumors) are contraindicated.

5、 Precautions

Indication for discontinuation: If uncontrolled hyperglycemia, significant increase of liver enzymes or severe water retention occurs during the treatment, the drug should be stopped immediately.

Individualized decision-making: All contraindication judgments should be made by the attending physician based on specific examination results, and patients should not self compare and stop medication.
