The indications for Voranigo are clear, mainly targeting patients with grade 2 astrocytomas or oligodendrogliomas who have undergone surgery, including biopsy, partial or complete resection, and who have susceptible IDH1 or IDH2 mutations.

1. Standard usage and dosage

Dosage: It must be taken on an empty stomach (at least 1 hour before or 2 hours after meals), swallowed whole, and taken with a glass of water.

Fixed dose: The recommended dose is once daily, 40mg each time (two 20mg tablets). It is recommended to take medication at the same time every day to maintain stable blood drug concentration.

Dose adjustment steps: If a reduction is required due to adverse reactions, follow the principle of decreasing dosage - gradually decrease to 30mg/day, 20mg/day, and 10mg/day; If unable to tolerate 10mg, the medication will be permanently discontinued.

2. Efficacy positioning

Applicable scenarios: This product is used to treat specific gliomas carrying IDH1 or IDH2 mutations (such as grade 2 oligodendroglioma or astrocytoma), and is used after surgery or radiotherapy and chemotherapy.

Treatment objective: To delay tumor progression and prolong progression free survival by inhibiting mutated IDH enzyme activity. The therapeutic effect needs to be comprehensively judged by combining imaging (MRI) and clinical evaluation.

3. Core precautions (safety first)

Hepatotoxicity monitoring: Regular testing of ALT, AST, and bilirubin is required during treatment. If jaundice, dark urine or severe fatigue occur, seek medical attention immediately.

QT interval prolongation: Regularly review the electrocardiogram to maintain normal blood potassium and magnesium levels. Avoid using other drugs that prolong the QT interval. If palpitations or fainting occur, seek medical attention immediately.

Differentiation syndrome: Be alert to sudden high fever, difficulty breathing, or sudden weight gain, which are considered emergencies and require immediate use of glucocorticoids and contact with a doctor.

Drug interactions: Strictly avoid grapefruit or grapefruit juice to avoid affecting drug metabolism.

Taboo population reiterated: severe liver and kidney dysfunction (eGFR<30), pregnant women should not use it. Men and women of childbearing age must strictly use contraception (2 months for women and 3 months for men after stopping medication).

4. Important Notice

All dose adjustments, concomitant medications, and adverse reaction management must be carried out under the guidance of the attending physician, and do not change the plan on your own. Regular follow-up is the foundation for ensuring safety and efficacy.
