Epclusa is a pan genotype antiviral drug for hepatitis C, but it is not suitable for all patients. Severe liver injury patients may have an increased risk of liver toxicity, while combination with certain antacids can significantly reduce efficacy. There are two aspects to clarify its contraindications and medication precautions.

1、 Population with severe liver injury (Child Pugh B/C grade)

1. This product is not suitable for patients with moderate to severe liver function impairment (Child Pugh B or C grade).

2. This population may increase the risk of liver decompensation, including jaundice, ascites, or hepatic encephalopathy, after use.

Before treatment, a Child Pugh score must be performed to clarify the liver function grading.

If there is a history of liver decompensation (such as variceal bleeding, refractory ascites), it should be avoided.

Mild liver injury (Child Pugh A grade) can be used at regular doses, but liver function changes still need to be monitored.

2、 The risk of combination use with specific antacids

1. Antacids containing magnesium, aluminum, and calcium (such as aluminum hydroxide and calcium carbonate) can reduce the absorption of Epclusa and lower blood drug concentrations.

If antacids must be used in combination, they should be taken at least 4 hours away from the time of taking this product.

Proton pump inhibitors (such as omeprazole) also need to be administered intermittently, and it is recommended to use them 4 hours before taking Epclusa.

4. H2 receptor antagonists (such as famotidine) can be used under the guidance of a doctor, but should not exceed the recommended dosage in the instructions.

5. If any stomach medication needs to be added during use, it should be consulted with a physician or pharmacist first to avoid self medication.
