Elacetrant is an oral selective estrogen receptor degrading agent (SERD), which is used to treat patients with ER positive, HER2 negative and ESR1 mutation in advanced or metastatic breast cancer. The recommended dosage is 345mg orally once daily. Attention should be paid to missed doses and dosage adjustments, while also being alert to adverse reactions such as dyslipidemia and embryotoxicity.

1. Usage and dosage

The recommended dose of Ailasetran is 345mg orally once daily until disease progression or intolerable toxicity occurs. If a reduction is required, it can be adjusted to 258mg for the first dose (three 86mg tablets) and 172mg for the second dose (two 86mg tablets).

Indications for ESR1 mutation

Elastigmine is applicable to advanced or metastatic breast cancer with ER positive, HER2 negative, ESR1 mutation in postmenopausal women or adult men, and disease progression after at least one endocrine therapy is required.

3 Precautions

(1) Handling of missed services

If the missed dose exceeds 6 hours or vomiting occurs, the dose should be skipped and the next dose should be taken at a normal time the next day. Drug overdose requires immediate medical attention.

(2) Dose adjustment

Grade 1 adverse reactions can maintain the original dose; Level 2 requires consideration of temporarily interrupting treatment; Level 3 requires interrupting treatment and reducing dosage after recovery; Level 4 requires interruption of treatment and permanent discontinuation of medication after recovery.

(3) Special populations

Pregnant women are prohibited from breastfeeding, and breastfeeding women should stop breastfeeding during treatment. Patients of childbearing age need to take effective contraceptive measures. Patients with moderate to severe liver function impairment need to adjust the dosage.

(4) Adverse reaction monitoring

Regular monitoring of blood lipid status is necessary to be alert to the occurrence of hypercholesterolemia and hypertriglyceridemia.
