Sorafenib has clear contraindications for use, especially in severe liver and kidney dysfunction and pregnant populations with significant risks. The following are the core taboos and precautions to guide clinical safety decisions.

1、 Severe liver dysfunction patients

Patients with severe liver function impairment (Child Pugh C grade) are contraindicated as the drug is mainly metabolized by the liver, and severe decline in liver function can lead to drug accumulation and increase the risk of liver toxicity.

During medication, liver function indicators (such as transaminase and bilirubin) should be regularly monitored. If there is a significant increase, medication should be suspended or the dosage adjusted.

Mild to moderate liver function impairment patients should use it with caution and under close monitoring.

2、 Severe renal insufficiency patients

Patients with end-stage renal disease or requiring dialysis lack safety data and are not recommended for use.

Severe renal insufficiency (creatinine clearance rate below 30mL/min) may affect drug excretion, increase exposure, and increase the risk of adverse reactions.

Baseline renal function should be evaluated before medication, and blood creatinine and urea nitrogen should be regularly rechecked during treatment.

3、 Pregnant women and women preparing for pregnancy

Sorafenib has potential teratogenicity and toxicity to fetuses, and is contraindicated for pregnant women.

Women with fertility must take effective contraceptive measures during treatment and for at least 2 weeks after discontinuing medication.

If pregnancy is detected during treatment, the doctor should be informed immediately to assess the benefits and risks of continuing medication.

4、 Other alert groups

Individuals allergic to sorafenib or its excipients are prohibited from using it.

Breastfeeding women should stop breastfeeding during medication to avoid drugs affecting the baby through breast milk.
