Trelagliptin is only applicable to specific people with type 2 diabetes. It is forbidden or cautious to use Trelagliptin in type 1 diabetes, ketoacidosis and severe liver and kidney dysfunction. Before medication, it must be evaluated by a doctor and cannot be purchased or adjusted on one's own.

1、 Absolutely prohibited population

Type 1 diabetes patients: This product is not applicable to type 1 diabetes patients. Because this type of patients rely on exogenous insulin to maintain their lives, this product cannot replace insulin, which may lead to uncontrolled blood sugar or increased risk of ketoacidosis.

Patients with diabetes ketoacidosis (DKA): no matter in the acute episode period or in the past medical history, it is prohibited to use. DKA belongs to acute metabolic crisis and requires emergency treatment such as insulin and fluid replacement. This product is ineffective and may delay treatment.

2、 The population that needs to be carefully evaluated or avoided for use

Severe renal insufficiency (eGFR<30mL/min/1.73m ²): This product is mainly excreted through the kidneys. In patients with severe renal impairment, drug clearance is significantly reduced, which may lead to blood drug concentration accumulation and increase the risk of adverse reactions. It is generally not recommended to use and other hypoglycemic regimens should be used instead.

Severe liver dysfunction (Child Pugh C grade): Severe impairment of liver function affects drug metabolism ability, with limited safety data, and is usually not considered as the preferred treatment.

3、 Necessary checks and communication before use

Before medication, it is necessary to test:

Islet autoantibodies (such as GAD antibodies) to exclude type 1 diabetes.

Renal function (creatinine, eGFR) and liver function (transaminase, albumin, bilirubin).

Proactively disclose medical history:

If you have a history of ketoacidosis, pancreatitis, or are currently using other hypoglycemic drugs, you need to inform your doctor.

Pregnant women, breastfeeding women, or those planning to conceive should assess the benefits and risks.
