During the use of Elafiprono, it is necessary to closely monitor creatinine levels, weight changes, and the risk of concomitant medication. Scientific monitoring can effectively prevent adverse events. The core precautions are listed in three aspects below.

1、 Management of elevated creatinine levels

Regular monitoring: Blood creatinine should be tested every 2 weeks during the initial stage of treatment, and rechecked once a month after stabilization.

Assess baseline risk: For those who already have chronic kidney disease (eGFR<60mL/min/1.73m ²) before medication, the benefits and risks need to be balanced.

Timely treatment: If blood creatinine increases by ≥ 30% compared to baseline, medication should be suspended and dehydration, infection, or other kidney injury factors should be investigated.

2、 Management of weight gain

Regular weighing: Weigh on an empty stomach every morning and record the trend of changes.

Dietary intervention: Control daily total calories, reduce refined sugar and saturated fat, and increase dietary fiber.

Moderate exercise: At least 150 minutes of moderate intensity aerobic exercise (such as brisk walking) per week can help reduce drug-related weight gain.

3、 Drug interaction warning

OATP inhibitors (such as cyclosporine and ritonavir): Combination use can significantly increase the blood concentration of Elafipronol, increase the risk of liver toxicity, and should be avoided.

CYP3A4 inducers (such as rifampicin and carbamazepine): may accelerate drug metabolism, reduce efficacy, and require dosage adjustment or dressing change.

Warfarin and other anticoagulants: Co administration may affect coagulation function, and INR monitoring needs to be strengthened.

Betrates and other lipid-lowering drugs: Combination use may increase the risk of myopathy or rhabdomyolysis, and muscle symptoms should be closely monitored.

When there is a significant increase in creatinine, rapid weight gain in the short term, or unexplained muscle pain, it is necessary to consult a doctor in a timely manner and not adjust medication or add new drugs on your own.
